Eligible patients can receive coordinated behavioral and medical care, known as your “Behavioral Health Home.” Your coordinated team includes a behavioral health provider, social worker and community health worker. The Minnesota Transgender Health Coalition and Rainbow Health Initiative have collaborated to produce a new updated LGBTQ-friendly health provider directory!  The purpose of the directory is to list all kinds of providers, from doctors and nurses to chiropractors and dentists, all of whom agree that they are personally and professionally supportive of the LGBTQ community. 

Family Tree Clinic

Family Tree Clinic’s mission is to cultivate a healthy community through comprehensive sexual health care and education.

They provide a wide variety of sexual health services and specialized care for LGBTQ individuals through all stages of life, including low-cost STI testing, limited primary care, and FREE HIV testing for all uninsured patients.

Reclaim MN 

RECLAIM increases access to mental health support so that queer and trans youth ages 12-26 may reclaim their lives from oppression in all its forms. We provide therapy to 65+ youth each week, 80% of whom are trans or gender nonconforming; develop practitioners with an emphasis on changing the landscape of access to care in our region through training POCI therapists in gender competent care; and partner in community through outreach & education and the intersection of gender and racial justice.

Resources for Your Providers

Unfortunately, sometimes your providers aren't as aware of trans-affirming care as we want and need. Here are some resources for healthcare providers to engage in further awareness-building about trans, gnc, and non-binary communities, particularly focused on serving trans+ patients.

WPATH Standards of Care (7th ed.) 

The World Professional Association for Transgender Health promotes the highest standards of health care for individuals through the articulation of Standards of Care (SOC) for the Health of Transsexual, Transgender, and Gender Nonconforming People. The SOC are based on the best available science and expert professional consensus.

The overall goal of the SOC is to provide clinical guidance for health professionals to assist transsexual, transgender, and gender nonconforming people with safe and effective pathways to achieving lasting personal comfort with their gendered selves, in order to maximize their overall health, psychological well-being, and self-fulfillment.

Transgender Training Institute 

Provides on-demand and live webinar training sessions on a variety of transgender-related topics. Previous sessions include:

  • Building Affirming Workplaces: HR Support for Trans and Non-Binary Employees 
  • Foundations of Awareness: An Intro for Clinicians (Mental Health)

Providers can also request customized training.
